Do not strip spaces from fontName specifications, as some font names do contain spaces. This requires perfect attention to spaces in fontName specifications, including the avoidance of spaces following commas. A better solution would probably be to normalize-space() each font name separately.

regenerated font metrics files for STIX 1.x; fixed a bug in ttfreader/TTFReader that sometimes caused fop's TTFReader to be used instead of ours, resulting in unusable font metrics files; renamed our modified TTFReader class to TTFReaderBBox to make this transparent

Changed xMax of ⎯ (horizontal line extension) from 766 to the correct 315. The wrong value was either caused by an erroneous font metric in a prior version of the STIX fonts, or there is a bug in ttfreader. To check, should rebuild ttfreader and rebuild the font metrics files.

Replaced named entities by numeric entities and moved the operator dictionary into its own namespace. Removed all references to the MathML DTD from the examples, and also from some stylesheets (where they were bogus).
